When it comes to buying gifts for the men in my life, I have a very hard time trying to come up with something that will be meaningful. I want to do something beyond the generic shirt, tie or handy man's tool. What kinds of successes have you had? Why is this so darn difficult?

I believe men are supposed to buy gifts for us and not the other way around.
(hehehe)
Men are impossible to buy for -- I usually just ask them what they want, straight up.

John loves gift cards to those Manly-Man shops. Cabella's, Bass whatever-it-is Shop, those places.
For his 50th b-day I got him a Cooper's beer making kit. It cost about $100. but he has had a lot of fun making beer. Also, I buy tools, really good quality Craftsman tool sets. He replaces his old cheap hand tools with the good ones. I have bought "big" tools like a table saw, chop saw, and stand grinder. Expensive high quality tools that he wouldn't buy for himself. I usually buy these at Sears, they have really good quality tools and if it turns out to be something he doesn't need he can return or exchange it easily.
For Christmas I often fill his stocking with small drill bits (they break easily and always need replacing), jig saw blades, sawz-all replacement blades and small grip-thingy's (what are they called?). Little hand tools stubby screwdrivers, stuff like that.
Last Christmas I got him a "hug light" it is a small 2-light flashlight with a flexable tube between the two lights. He wraps it around his neck when he is sleeping in his hunting tent. It is available and conveinant if he has to get up in the dark.
This year I might but him a blue-tooth for his phone. But am thinking also about a wine making kit. Just a few ideas I had.

Oh, something else, I bought John a new wedding ring a few years ago. AND a couple of times I bought him really nice pocket knives. He has a thing about fancy knives, I get my jeweler to order them in for him.

Michael doesn't wait for b-days/x-mas, he just goes and buys "what he wants-when he wants it". Then I say Happy Whatever!!

Last Christmas, when I didn't know my new son-in-law very well, I got him a gift card at a restaurant near the movie theatre, and movie passes. He thought it was a great idea.
He's a great gardener, so he got a gift card for the local nursery for his birthday.
One thing I like are the gift cards that are good for a whole mall, instead of just one store. It increases the possibilities for getting it right.

My Dad's a big fan of getting cash. He's not one to buy random things. He likes to hunt and fish, but he knows what he uses for that, and stuff for his shooting matches is usually out of my price range. He likes cash that he can use at the grocery store or the gas station or wherever.
My brother will usually just tell me what he wants if I ask him. His interests are generally limited to airsoft and his Cobra, but there is usually some kind of car-cleaning device he'll want or Under Armour for his tournaments and such.
I don't really try to come up with surprises anymore.
Libby, I agree with Ruff! John's a lucky duck!
